Version 2.2002

Made 1 formatting change to main version of EPIC to fix a value that got pushed onto the following page.

Version 1.2002

Changed the main version of EPIC to be the version without SF12 and AUASI and created a new version of EPIC, called EPIC-AUASI-SF12-1.2002.pdf, which is the version of EPIC with the SF12 and AUASI included.

Version 7.2001

Added the optional name and DOB fields to the front of each instrument, and added the single item satisfaction section back to the main EPIC instrument that had been removed in version 3.2001.

Version 3.2001

Removed the demographic questions from the main version of EPIC and created a demographic add-on instrument.

Version 2.2001

Updated Item 88 to clarify that radical prostatectomy referred to surgery to remove the prostate through the perineum or by using a laparascope, in addition to surgery through the abdomen. Changes made to EPIC only (No changes made to single domain versions).
